B4A Teens Conference Waiver & Release

PLEASE READ CAREFULLY BEFORE PROCEEDING.
By proceeding to and completing the B4A Teens Registration, you agree to the following terms and conditions:

As Participant (Teen Attendee):

  • I agree to follow all rules and guidelines set forth by Beauty From Ashes (“B4A”) Conference staff, volunteers, and mentors.

  • I agree to respect the safety, dignity, and well-being of myself and others during all B4A sessions and activities.

  • I understand that if I need to leave during scheduled program times, my parent/guardian must contact the B4A registration team directly.

  • I accept full responsibility for any personal belongings brought with me to the conference.

  • I acknowledge that participation in conference activities may involve physical movement and group interaction. I fully accept responsibility for any injury or accident that may occur while I am participating in the B4A Teens Conference.

  • I agree that in the event of an accident or illness while participating in the program, B4A, its staff, and volunteers are hereby released from any and all liability.

Photo & Media Release:

I agree and acknowledge that photos and videos may be taken during the event. These may be used by Beauty From Ashes for promotional, educational, and ministry purposes in print, online, or social media, without further notice or compensation.

As Parent/Guardian:

  • I give permission for my child to attend and participate in all activities of the B4A Teens conference.

  • I agree to follow all rules set forth by the Beauty From Ashes conference regarding communication and supervision.

  • I agree that if my child must leave during scheduled program times, I will notify the B4A registration team by phone. (The conference contact number will be provided in the confirmation email.)

  • I acknowledge and accept that B4A, its officials, staff, and volunteers are released from any liability for any injury or accident that may occur during my child’s participation.
